Rider Waiver Form

EVERY Park rider is required to  complete & sign a RIDER WAIVER FORM before entering the Indoor-Park

You can click the link below to complete the Waiver Form On-Line or fill out In-Store

If you are under 18 you need a parent or guardian to sign for you. The parent/guardian, the rider and an employee of The Village Park & Supply must be present during the signing of the waiver or confirmation of a completed online form by the nominated guardian must be obtains by a staff member.

A Waiver is only requires once per year or whenever your personal details change. Once a staff member has confirmed your waiver it will be securely saved for future reference so next time you visit all you need to do is give us your name.
